Skilled Nursing Services

Description of Nursing Services to be provided by RN or LPN's under Supervision of RN:

  •  Colostomy care and urinary catheter maintenance are essential aspects of home    health care.

  •  Personal medical care includes services such as nail trimming, oral cavity cleaning, and bowel program supervision, all vital components of nursing care.

  • Administering medication encompasses various methods, including injections, inhalants, eye drops, oral, and topical medications.

  • Setting up and maintaining a medication schedule, which may involve weekly pill boxes or automatic dispensers, is critical for patient adherence.

  •  Tracking oxygen levels and using heart monitors or other advanced in-home equipment are part of comprehensive home health care.

  • Providing advanced care training for family members ensures they are equipped to assist with nursing care needs.

  • Wound care should be performed according to Physician's Orders to promote healing and prevent complications.

  •  Assisting clients with ambulation supports their mobility as they walk from one place to another.

  • Transfer assistance is necessary for helping the client move from a bed to a wheelchair or vice versa.

  • Proper positioning of clients is crucial for those who are immobile or have appliances or casts that do not fit comfortably in the wheelchair.

  • Observation of the client for any changes in condition, including signs of pain, changes in appetite, weight fluctuations, and variations in vital signs or complexion, is essential in patient assessment.

  • Assessment involves a nurse identifying the needs, preferences, and abilities of the client, incorporating interviews and observations, as well as considering the client's medical and social history. Physical aspects evaluated include vital signs, skin color and condition, motor and sensory nerve function, nutrition, rest, sleep, activity, elimination, and consciousness.

  • Ventilator care focuses on reducing or eliminating dependence on the ventilator for breathing, contributing to effective home health care.

  • IV Therapy involves the infusion of liquid substances directly into the vein to provide necessary treatments.

  •  Diabetic care and education are vital for those living with diabetes and their families, as well as individuals at risk of developing the disease.

  •  Lab blood draws are performed to monitor various health conditions.

  • Injections are administered as part of ongoing nursing care.

  • Tracheostomy care includes suctioning the tracheostomy tube and changing the dressing to maintain airway patency.

  •  Enteral/parenteral feedings involve care and teaching for the delivery of a nutritionally complete feed directly into the stomach, which is essential for clients unable to eat normally.

  • Post-operative recovery care provides services based on doctor's orders following an operation, ensuring a smooth recovery process.
